Our fees are determined by several factors including contracts with insurance companies and the Federal Register. Per HMO and PPO contracts with insurance companies - all copayments are due at the time of service. If prior arrangements have not been made, payment in full (just your copay if you have a copay plan) is due at the time of service.

We would be happy to discuss financial arrangements with you and we often set up payment plans, etc. If you prefer to speak to our office manager directly and in confidence - please do. Her name is Cheryl and she is available via e-mail or phone. Please don't ever let financial issues stop you from seeking healthcare at our office.

Our office has contractual agreements with most insurance companies. Feel free to discuss insurance submission arrangements with our front office staff. We accept assignment from Medicare. You will be responsible for your deductible and 20% of the approved amount. You might also keep in mind that occasionally there are services not covered by Medicare for which you will be responsible.

Reports to insurance companies, attorneys or additional forms will constitute an additional fee. This fee is charged to the company involved but please be aware that occasionally they pass it on to you.

As with most businesses, we assess a service charge for returned checks.
